- The Exceptional Savings Plan is a dental savings plan and is NOT dental insurance.
- Membership fees are due in full at enrollment and are non-refundable.
- Membership benefits become active upon enrollment and remain valid for one year from the enrollment date.
- Membership benefits are only valid at Exceptional Dentistry and are non-transferable.
- Membership cannot be combined with any other dental insurance, discount plan, promotion, special offer, or reduced-fee program unless otherwise authorized by Exceptional Dentistry.
- Membership fees and benefits are subject to change upon annual renewal.
- Treatment must be paid for in full at the time of service to receive membership savings. Any unpaid balances may be billed at regular office fees.
- Membership benefits may not be applied retroactively to treatment completed before enrollment.
- Membership is not transferable to another individual, provider, or dental office.
- No refunds or partial refunds will be issued if a member chooses not to utilize any or all plan benefits.
- The plan does not cover services not specifically listed as included benefits.
- In the event that a member terminates treatment, relocates, or seeks treatment elsewhere, membership fees remain non-refundable.
- The plan cannot be used for treatment associated with personal injury claims, workers’ compensation claims, automobile accident claims, medical reimbursement claims, litigation, or third-party liability cases.
- The treating dentist reserves the right to determine treatment recommendations and eligibility for services based on clinical judgment and standard of care.
- Membership benefits expire at the end of the membership term and do not roll over into future years.
- Exceptional Dentistry reserves the right to modify, discontinue, or replace the membership program upon renewal periods.
